Dover COVID-19 vaccines for children ages 5-11

The Dover School District and Dover Fire are collaborating to offer the Pfizer COVID vaccine for children ages 5-11.

Dover Schools and Dover Fire would like to know your needs and preferences for vaccinations - we are currently planning for the week of November 15th.

THIS FORM MUST BE FILLED OUT BY 3 PM ON TUESDAY, NOVEMBER 9.
